Description
The job function of a Case manager is to enhance the quality of patient management, maximize patient satisfaction and promote cost effectiveness working in collaboration with specialists and claims team.He/she will be accountable for the full patient care cycle.Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the clients health needs from diagnosis to treatment to post discharge, in collaboration with specialists and claims team. Requirements
ExperienceWorked in Acute Tertiary hospitals in Singapore and specialized in Critical Care, Emergency Care or Operating RoomsHeld senior nursing leadership position for more than three years (Nurse Manager or Nurse Clinician)Proven working experience in case management, including as a nurse manager or related jobExcellent knowledge of disease managementFamiliarity with professional and technical emerging knowledge. Proficient in patient education & counsellingResponsibilitiesDevelop SOPs on case managementDevelop the process of case management; both internal and external referral workflowDevelop strategies to market case management as a value added service; both internal and external clientsExplore disruptive innovation and web-based platform Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centeredHandle case assignments, draft care plans, review case progress and determine case closureFacilitate multiple care aspects (case coordination, information sharing, etc)Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment optionsDevelop effective working relations and cooperate with specialist providers throughout the entire case management processTake the extra mile and interact with patients to keep track of their progress and to ensure satisfactionRecord cases information, complete accurately all necessary forms and produce reportsPromote quality and cost-effective interventions and outcomesAdhere to professional standards as outlined by protocols, rules and regulationsGrow the Case Management service and train new membersQualifications
Skills & KnowledgeExcellent people management skillsEffective communication skillsExcellent organizational and time management skillsProblem solving skills and ability to multi-taskCompassionate with teamwork skillsLicensesCurrent & valid SNB licenseEducation LevelDiploma/Advanced Diploma/Bachelor/Master degree in NursingAdvanced Diploma in Critical Care/Oncology/Renal Nursing is highly preferred
