Plan, co-ordinate and implement the Community Networks for Seniors (CNS) operations at the GRC level to integrate the health and social services to support seniors holistically
Assist in developing and implementing data collection systems that includes a needs database to identify and track the needs of seniors living in each community
Interpret data analytics to develop data-driven strategy to support the CNS pilot operations, and to support outreach efforts
Care coordinate referrals (including making calls and home visits) of clients living in the community who need health and/or social services and adhere to the personal data protection act requirement established by the organisation
Manage and monitor project plan, project milestones, risk and change management to ensure delivery of the project on schedule. Ensure CNS pilot operations meet key objectives and timelines
Review pilot performance and outcomes for continuous improvement to increase operations efficacy and efficiency
Job Requirements
Recognised degree in any discipline
Minimum 2 years working experience preferably with allied health or community engagement background
Strong interpersonal and communication skills
Project management and planning skills
Possess good critical thinking, analytical and decision-making skills
Strong in follow-up work and is able to mobilise people into action
Passion for working with seniors and community partners
Ability to work in a fast-paced and dynamic work environment
Able to attend evening meetings and weekend events in collaboration with key partners

Handling end to end New business processing starting from creation of Client id,Case start up, New business login, Follow up for policy issuance, Quality Check
Follow up with HUB for policy issuance of pending cases
